A local restaurant we go to has a little advertising billboard for rent right at eye-level right above the urinals in the mens room. It's hard to ignore (especially we guys tend to stare straight ahead anyway...it's in the male handbook...you don't look side to side and talk to your neighbor).
Here's another example of bathroom advertising in Thailand, pitching Scott brand tissue paper:

Landing the deal means approaching your customers creatively. Where they can't look away. Not getting results from your cold calling, telemarketing efforts? Time to adopt a new, fresh approach. Talk to your customers where they aren't expecting you to...where their defenses (and possibly their pants) are down.

I absolutely agree that new marketing efforts have to be creative. Advertising space above the urinals is a perfect place where to advertise. Unfortunately, it's incredibly expensive as well, since restaurant owners are already aware of the "restroom value." As for technical side of advertising in the restroom, I would go for as creative commercial as possible. Which brings me to an idea that bathroom (be it ladies or gents) is a great place where one can advertise products with cringe factor (condoms etc.) Just think about it.
